Would Elizabeth be able to teleport directly inside the warehouse through the broken window? I though she had to be able to see exactly to where she was teleporting.

Yes, you can spend a HP on your turn as a Free Action. You can only use an HP one time per round for any particular use, i.e. only one reroll per round, one recover a level of Fatigue, etc... But you can spend multiple HP in one round for different uses, e.g. use one HP to reroll and another HP to recover Fatigue in the same round.

Is Elizabeth unable to see the person/thing from which the voice is emanating? Does she need a Perception roll?

Kodiak doesn't smell anything too unusual. You are in an abandoned office building, so there is the odd smell or two, but nothing that seems dangerous. Probably just old cleaning items or the like.

You blocked the teleport before using a radio jammer so the teleporter couldn't get a lock on to them. Madame Macabre's magic is not capable of anything like that. She might be able to reverse her own teleport spell to block a magical one, but there's is not magical.

Nathan doesn't detect anything unusual on your comm frequency.

He isn't hiding anything. He still thinks you are wasting his time. For whatever reason, your mention of magic didn't impress him at all. He might not believe in magic, or it could be something else.

The team communicators have GPS in them. So its fairly easy to track any of the team members as long as their communicators are online.

Madame Macabre would know that due to the way Khan exited the world, this would not be a "standard" resurrection. They would need to open a portal to where he got sucked to. And while its possible to open one just to bring back Khan, anything that survived there for this long would no longer even remotely be human.
